Please answer the following questions to initiate the mass booklet design process. Please pay particular attention to spelling of individuals included in the mass booklet as well as a description of their relationship to the deceased in the exact wording you prefer. For example, “a reading byJean Smith, John’s sister.”

If you are planning a different type of service, please reach out to me via my contact form so a custom program can be created to suit your needs. Thank you.

Programs need to be completed and printed at least 48 hours prior to the service date in order to ensure on-time delivery. Once the form below is received, a design proof can be produced within 8-12 hours. Therefore, I ask for the form below to be submitted at least 72 hours prior to the service date to allow for digital proof review, ongoing design consultation, and editing time.

The earlier the form is completed in this process, the better. If your timetable does not allow for this, please reach out to me directly to discuss options.

Upon final approval of the design, an invoice will be emailed to the primary contact for payment based on the quantity pricing outlined below. Mass booklets will be delivered to the funeral home.
